ICTSI’s Manila terminals strengthen incident investigation and preventive action planning capability


 

As part of efforts to ensure that investigations of serious incidents are comprehensively conducted, the ICTSI Global HSSE Department organized an Incident Cause Analysis Method (ICAM) Lead Investigator Course for select employees of Manila International Container Terminal, Manila NorthPort, Manila Multipurpose Terminal, and the Corporate Office.

ICAM is an in-depth investigation process that goes beyond identifying immediate causes. It digs deeper into determining contributing factors, underlying root causes, and failures within the broader organizational system—such as operating procedures, equipment, training, communication, conflicting goals, and organizational culture—that led to an incident. Through a systematic analysis of this information, ICAM enables investigators to identify what truly went wrong and make fit-for-purpose recommendations for corrective and preventive actions to reduce risk and build error-tolerant, resilient defenses against future incidents. The ICAM process also incorporates best practices in Human Factors and Risk Management principles.

Twenty-five employees from the HSE, operations, and engineering departments participated in the two-day course held from 25 to 26 November. Facilitated by ICAM International from Australia, the training equipped lead incident investigators with a practical understanding of the investigation methodology, as well as the knowledge and skills required to effectively lead and facilitate incident investigations.
